How could it have "just failed" if it works reliably at the login screen but not after login???

Try creating a new user account (I would say to call it "test" but know that it may be a little out of the question for you)

After that, log out and log back in with the other user account and see if it will work reliably with the "S" key.

If it does, then you likely have some profile corruption. At which point, I would open a terminal and enter "rm -Rf .*" and then "cd Library/Preferences && rm -Rf com.apple.system*"

That will remove the user-specific input locale configuration files that may have become corrupted. Upon the next login, they will be replaced with the copies that exist in /System/Library/Preferences" that allow the "S" key to work before logging in. User specific settings override the System-wide settings after login.
