My ga-ep45-ds3® refuses to boot when the UL5D is in (tested in the 8x and the 16x). I put it back in the mac pro, flawless. I also have UL3D - no love there also. I also have UL3D - no love there also. Can you get an ioreg -l output with it in the mac pro? (And possibly email me a copy of the driver?) I'm trying to get a LSI 320-2e card working...., it might help, or I may spot something obvious.
