UHD 630 Not Outputting 4K

Hey all. I just completed my first Hackintosh build with OpenCore. Pretty much everything is working as expected except my monitor (HP Envy 27" 4k) is only getting 2560x1440 max instead of 4k from the integrated graphics. More details and specs below:  


Things I have done 
1) Applied framebuffer patches, increased VRAM, attempted to increase DVMT although I'm not sure it's an option with my motherboard.  
  
2) Switched from HDMI cable to USB-C cable as output to display (don't have a displayport cable but I think USB-C outputs as DP)  
  
3) Used both SwitchResX and ResXtreme – neither give the option for 4k
  
4) Held option down when selecting scaling in Display settings

and YES, my motherboard supports HDMI 2.0, etc necessary for 4k.  
  
Build Info
Motherboard: ASRock Z390 Phantom IV  
CPU:Intel i9-9900k  
RAM: 2x 8GB Corsair Vengeance 3000MHz  
SSD: Samsung EVO 970 Pro 500GB  
SMBios: iMac18,1  also tried Macmini8,1

I've made changes to this in my attempts to apply framebuffer patches that haven't worked, but this is my original.  
  
config.plist (serial number,etc removed – not sure if it matters)
 

I've googled and read just about every thread on Reddit or here about UHD 630 and nothing has worked.
