You need to patch AppleIntelGMA950.kext and AppleIntelIntegratedFramebuffer.kext

Find 86 80 A2 27 and replace it with 86 80 A6 27, this should get your primary monitor working.

You will need to do exactly the same for your external monitor but I can't find what you should replace. You need to insert 86 80 A6 27 in place of the default apple secondary output (trying to find the value for you atm).
