Video output problems with 4k samsung monitor when switch between sources

Hi all (and maybe sorry for my English), 

I've installed Mojave 10.14.6 on my Hackintosh build, everything seems working fine except the video output. 

The computer is connected to my Samsung Tv 4k series 5 via DisplayPort - HDMI cable on port HDMI2/DVI.
When I turn on the Hackintosh, Mojave boots up correctly and I see the desktop after UefiBIOS and Clover, but when I switch to source HDMI3 or 4 and so on and I go back to HDMI2/DVI the tv shows "no signal" message, but the computer is still working properly because if I connect via VNC I can use it. 

The only way to exit to this impasse is to unplug and plug the cable back and sometimes screen turns on. 
Other times when this trick does not work, the only way is to shut down with the power button and power on... then go back to work. 

The same issue happens when I switch off the tv with computer running and turn on tv ... it's seems like de video card (AMD RX 560 4GB) or the system(?), does not recognize the tv. 

I hope someone can help to solve this issue.

PSU EVGA Supernova 450 GM
iogear GBU521 W6 Bluetooth USB
Case NZXT H210
Radeon RX 560 4 GB (soon I will replace with RX 580 8GB)
