Wireless (D-Link WDA-1320) works about 10 percent of boots..?

Greetings and thank you in advance for your help. I'd appreciate any I can get, as I am genuinely stumped.

I'm absolutely new to the Hackintosh scene, but I have some computer experience in other areas. I've gotten to a point where I have full graphics support. Still a kernel panic on every shutdown, but that's something I plan to address later.

My process so far:

1. Vanilla install, dual boot with Windows 7. I used ##### to boot to the retail SL install DVD. Partitioned into two partitions using GUID Partition Table, formatted first partition as Mac OS Extended (Journaled), second as FAT. Exited SL installer, booted to Windows 7 install CD, reformatted FAT partition to NTFS and installed Windows 7. Booted to SL installer using ##### again, and installed SL without printer drivers.

2. On first boot, I booted using ##### to the new SL partition, updated to 10.6.6, and without restarting, used ##### (#####, System Utilities, EvOreboot, NullCPUPowerManagement, Voodoo PS/2 Controller), and then added the device ID of my graphics card to S/L/E/NVDANV50Hal.kext, then used Kext Utility to make Extensions.mkext. Enabled verbose boot in E/E/com.apple.Boot.plist Kernel Flags.

3. Restarted and booted repeatedly through Chameleon (which was installed by #####).

This is the weird part. About 1 in 10 boots, the system boots to a state where the wireless card is fully working. The other nine times, it's not recognized. I'm not changing anything or touching anything between reboots, except for using the mouse to reboot. kernel.log is slightly different on boots where the card loads:

When it loads, these lines are unique:

kernel[0]: Atheros: mac 7.8 phy 4.5 radio 5.6
kernel[0]: AirPort_AthrFusion: Ethernet address 00:24:01:03:16:a0
kernel[0]: IO80211Controller::dataLinkLayerAttachComplete():
adding AppleEFINVRAM notification
kernel[0]: IO80211Interface::efiNVRAMPublished():

And when it fails, this line is unique:

kernel[0]: USBF:	0.736
AppleUSBUHCI[0x7083000]::start unable to initialize UIM

The card has worked perfectly on Windows and Ubuntu for the past two years, and continues to work now when I boot into Windows 7 and Ubuntu. Interestingly, the 10% behavior also applies to the SL installer DVD loaded through ##### -- about 10% of the time, the card shows up in the installer DVD's System Profiler.

I'd appreciate any insight you veterans could offer on this issue. :)
