Wake from sleep fails when using 2nd screen Radeon RX Vega

Since connecting 2nd screen to my setup the wake from sleep is not working anymore. 

 

Setup:
2nd Dell U2515(1440p) -> HDMI -> Port 3 from above Sapphire Radeon RX Vega 64 Nitro (Tried DisplayPort cable and port, no change). 
1st Dell P2715Q (4K) -> DP -> Port 1 from above Sapphire Radeon RX Vega 64 Nitro

System enters sleep correctly. Wake initialise seems to be ok as well. U2515 displays correctly shows the welcome screen and any notifications, for a few seconds. Screen starts to flickr with sometimes 2 red horizontal lines. The P2715Q is quite slow and during this process it didn't show some output. After that both screens turn black. However there is still input signal. In the past wake from sleep resulted in hangs or so and also screens entered to sleep mode. This is now not the issue. Signal is there. When a new email arrives I can hear sound too. Also IP address and shares are reachable over the network. The P2715Q is quite slow. When my PC started in the past from a cold boot it was possible that clover was already booting the last OS after a 3sec delay without any output on the screen. In OSX when the 2nd boot stage was displayed (the white bar under the apple logo) the screen was initiated eventually. Now with a 2nd screen it seems that the U2515H is always faster and displays immediately the clover boot screen when I do a cold boot. It changes when I do a reboot. Then the P2715Q is the first screen displaying the clover screen.

If I remember correctly. When I choose to prevent the computer from sleeping under power safe settings the system can behave the same wakening the screens when the system was not even went to sleep mode.

It looks like that the hack doesn't initialises the 2 screens correctly. 

I also notice slowdown messages in "pmset -g log | grep -I slow"
