Need Help, Also Pretty Launchpad Bug

Hi guys. Thought'd I'd share this screenshot I nabbed of my Macbook's Launchpad freaking out a little. It sort of kaleidoscoped the icons somehow.

Also there are some apps that simply refuse to show up under Launchpad. Mainly, either Call of Duty 4 Single Player will show up, OR the multiplayer app. never both. Any way to manually add/remove icons in Mountain Lion's Launchpad?
