You can't shutdown/reboot after waking up from sleep or you need to wait on a black screen ?

I can confirm putting vanilla AppleSMBIOS.kext (from 10.5.2 in my case) fixed EVERY reboot/shutdown bug I had. Now I can shutdown/reboot after waking up from sleep, which I never could.

It seems to affect NVIDIA users mostly, so here it is.

The only downside of this is your CPU frequency won't get detected correctly, and system profiler won't report your hardware.

I think some apps won't work anymore (like Photoshop CS3 but i need to reinstall it to be sure) but DVDPlayer.app still work fine there...

I think the bug is due to smbios correctly reporting CPU frequency : because with vanilla SMBIOS I can now see in verbose mode when I shutdown : "CPU Halted", which I couldn't see with any HACKED SMBIOS...

If this is the bug then I guess we can use all other fixes from it (to fix app crashes, etc but again, i'm not sure I didn't try CS3 !) except the CPU frequency thing...

It's might not be the solution you were expected but AT LEAST there is a workaround and we know where it comes from.
