That happens when the icon in the Dock cannot find the application that it represents. Either because it no longer exists or it got moved to a different location. Add an app to the Dock and then go move the app to another location. You will get a question mark. If he installed the OS through Pacifist and didn't bother installing the Applications folder along with it, then none of those apps in the Dock can find the app they represent.
