X99 Hackintosh with Thunderbolt wire in card won't boot

Update:  I got my computer to boot - I'm so close - but can you please have a look to see what I might be missing:

I built the system in October and only recently installed a Thunderbolt EXII card to use a BlackMagic Intensity Shuttle (thunderbolt)  The device works fine in Windows 10.

Memory Allocation Error:
-Fixed the memory allocation error using a modified efi - OsxAptioFixDrv-99

Win10
-Installed Windows 10 on seperate SSD
-Got the card working with the ASUS drivers, in the mini SB Slot 4x_1. (I also got it working in the NB x16_4 slot, but it works best in the SB slot with my other cards (Firewire, esata, and Nvidia 980). Otherwise a card is right below the Nvidia fan.
-I *think* I updated the firmware properly, although the .bin file I updated was in the downloads folder - I know little about Windows, lots about real Macs.

EFI on Boot Drive:
-Removed all '32' files in the EFI
-Removed all blank directories in the EFI.

BIOS:
I have a Asus x99 - Deluxe 3.1 board - Bios Revision 3004,
5820 intel i7.
32 Gig Ram.

Once it worked in Windows, I set it to these settings for OSX per instructions I found online:

TBT Device IO resource Support: OFF
Reserved Mem per phy slot: 128
Reserved PMem per phy slot: 128

The device still is not detected.
I have it configured as a Mac Pro 5,1 and under Thunderbolt it says "no drivers are loaded" if that helps at all.
